Sample Release Form: General

If you want to be able to use workshop particpants' stories as examples in the future, you need to have them sign a release form. This is a basic release form used in the past at Center for Digital Storytelling open workshops. We print the release form on our letterhead and ask participants to sign it at the end of the workshop, if they are willing to give us permission to use their story in teaching and promotional activities.

This form is a Word Doc so you can fill in the blanks spaces with your organization's information.
