This form allows participants to choose from a list of release options. For example, a participant can choose to share his/her story as a teaching tool in workshop settings, but not to have it posted to the web or shown at conferences.

It gives participants the option of sharing their story with us for teaching purposes, conferences, and promotional materials, as well as on our website. We print this form on our letterhead and pass it out at the end of the three-day workshop.
